Past Graduate Response form
Use this form if you didn't complete the online Graduate Response when you finished your academic program, and now need to claim your print testamur.

Replacement Testamur Request form
Use this form to request a replacement print testamur. If you completed your academic program between (and including) Semester 2, 1991 to Semester 1, 2017 to request a digital testamur. If you completed your academic program after Semester 2, 2017, you have already been issued with a digital testamur which is hosted in the secure My eQuals portal. This step by step guide will assist you.

Change of Name & Personal Details form
Use this form if you are a past graduate and unable to make an online Name Change Request via your Student Centre.

Authentication of UniSQ Graduation Documents Request form
Use this form if you are submitting original (not copies of) print official graduation documents for authentication by UniSQ officials.

Verification Letter Request form
Use this form to request a letter confirming the basic details of your completed academic program. You may find the Third Party Verification of UniSQ graduates service is a preferable method for third parties i.e. recruiters, employers, professional bodies etc. to confirm you are a UniSQ graduate.

Ceremony Invitation Letter Request form
Use this form if you are a graduand/graduate who has already registered for a current graduation ceremony and paid the Ceremony Fee. This letter can be used to support your own application for a visa, but also to assist your guests to apply for a visa.
